The Application must be accompanied with the documents contained in "What documents are needed?".

Common documentation for qualification and competence certificates

• Identification document
• Passport photo (one)
• Medical certificate for seafarers (issued by a doctor on the list of doctors recognized by the Directorate-General for Health)
• Maritime note (cover sheet and records of shipments and landings, duly stamped and initialed)
• Proof of sea service (when applicable)
• Basic Security Certificate
• Protection Awareness Qualification Certificate
• Course or exam certificate (when applicable).

For certificates of competence:

Deck section

• Training log book (when applicable)
• Certificate of Competence (when applicable)
• General Operator Certificate at GMDSS (operational level)
• GMDSS Restricted Operator Certificate (coastal - management level)
• Radio Operator Certificate in national GMDSS A1 / A2 (coastal - operational level)
• Qualification certificate for the operation of lifeboats and rescue boats
• Qualification certificate for the control of firefighting operations
• Qualification certificate to provide first aid on board vessels
• Qualification certificate for resp. for healthcare on board vessels (management level)
• Qualification certificate for the exercise of specific protection functions
• Qualification certificate for ship security officer (management level)
• Training Certificate in Bridge Resource Management (BRM)
• Certificate of training in ECDIS
• Arpa Certificate in Simulator.

Machinery and electrical engineering section

• Training log book (when applicable)
• Certificate of Competence (when applicable)
• Qualification certificate for the operation of lifeboats and rescue boats
• Qualification certificate for the control of firefighting operations
• Qualification certificate to provide first aid on board vessels
• Qualification certificate for the exercise of specific protection functions
• Training Certificate in Resource Management at Casa da Máquina (ERM)
• Training certificate in Maintenance and repair of electrical, electronic and control systems (SEEC) (when applicable)
• Training certificate in Operation and Safety Management of Electric Power Installations above 1000 V (when applicable).
