The request is made through a Request, available at "Execute Service", and sent to the e-mail address dpm.requerimentos@dgrm.mm.gov.pt
The Application must be accompanied with the documents contained in "What documents are needed?".
For all types of exam:
- Identification document
- Passport photo (one)
- Medical Certificate for Seafarers (issued by a doctor on the List of Doctors recognized by the Directorate-General for Health)
- Maritime note (cover sheet and records of shipments and landings, duly stamped and initialed)
- Basic Security Certificate
- Protection Awareness Qualification Certificateão.
By type of exam for obtaining a qualification certificate:
Rule II / 5 or III / 5
- Qualification certificate for navigational watch service or engine watch service.
Rules II / 4, II / 5, III / 4, III / 5 or III / 7
- Training log book (when applicable)
- Course certificate suitable for Sailor or Driver or Electrotechnician (ETR).
Rule II / 4 or III / 4
Declaration by the master / company (1) indicating that he has performed duties in the service of navigational quarters or machines with a duration of not less than:
- 2 Months, in the last 5 years (with driver or sailor course)
- 6 Months, in the last 5 years (without a driver's or sailor's course).
Rule III / 5 or III / 7
Statement by the master / company (1) indicating that he / she performed functions in the machinery or electrotechnical (ETR) section of a duration of not less than:
- 6 Months, in the last 5 years (with a driver's or electrician's course)
- 12 Months, in the last 5 years (without a driver's or electrician's course).
Rule II / 5
Commander / company statement (1) indicating that he / she has performed in the deck section, of duration not less than:
- 12 Months, in the last 5 years (with sailor course)
- 18 Months, in the last 5 years (without a sailor course).
(1) Unnecessary if completed on-board training log book is presented
